If you are happy to share any health information with us, we will use this to provide relevant information and support, monitor our services and make improvements in future. In order to keep everyone using the Young Adult Service safe, we ask for emergency contact details for anyone under 18. These will only be used in case of emergency if we are worried or concerned about you.
